Reporting Safeguarding Concerns

If you have any safeguarding concerns, please report them here. Your report will be treated with the utmost confidentiality and urgency. Safeguarding is a top priority, and we encourage staff to promptly report any issues to ensure the safety and wellbeing of all service users.

Safeguarding concerns may include:

  • Suspected abuse or neglect

  • Concerns about a service user’s safety

  • Any form of harm or exploitation

Please provide as much detail as possible, including the full name and postcode of the individual involved. This will help us address the concern swiftly and appropriately.

Your vigilance helps us maintain a safe environment for everyone.

Incident Report:

Instructions for Reporting a Safeguarding Concern

  1. What to Report:

    • Nature of the Concern: Clearly state what the safeguarding issue is (e.g., suspected abuse, neglect, exploitation).

    • Details of the Incident: Provide a detailed account of what happened, including:

      • What you observed or were told.

      • Any concerning behaviour or actions.

      • The location of the incident.

  2. Who to Report:

    • Service User Information: Include the full name of the individual involved.

    • Postcode or Address: Ensure the correct postcode or location of the service user is included.

  3. When it Happened:

    • Provide the date and time of the incident or when you became aware of it.

    • If the concern is ongoing, mention this clearly.

  4. Who Else is Involved:

    • If there are other people involved (staff, family members, other service users), include their names and roles, if known.

  5. Action Taken:

    • Mention if you have taken any immediate action (e.g., removed the service user from harm or notified a manager).

  6. Amount of Detail:

    • Be as specific as possible. Include factual information and avoid assumptions or guesses.

    • Describe the incident in clear, simple language, focusing on what you know directly.

  7. Confidentiality:

    • Keep the information confidential and share it only through the proper reporting channels (e.g., the safeguarding lead or manager).

By providing accurate and detailed information, you help ensure that the safeguarding concern is addressed promptly and effectively.
