Your Sunlight Care Journey

1. An initial chat

When you’re looking for care for a loved one, it’s easy to feel like you’ve got nowhere to turn. Our first job is to listen to you about what you’re experiencing and what you want for the ageing loved one in your life. From there, we can help put together the right package of care for you and your family.

We will gather some details from you about the person who is looking for home care, and arrange a good time for us to visit and carry out a full Care Assessment with you, your relative(s), and anyone else who is important, such as advocates or friends.

2. Your Sunlight assessment

A Care Assessment is a thorough assessment of care and support needs.

​

An experienced member of our team will visit the person looking for care and support in their home to complete the assessment with you, your family and anyone else who is important such as advocates or friends.

​

During the Care Assessment we will ask you to share what is important to you, find out about your life history, health needs, daily routine and lifestyle. We can then work with you to create a care plan that sets out how we will support you to achieve your goals and remain independent in your own home.

3. An introduction to your

Sunlight Care Professionals

Using the knowledge from your initial Care Assessment, we can pair you with a Sunlight Care Professional/s who not only is the best choice for supporting your care needs, but who you can connect with on a personal level.

Feeling comfortable in your own home means knowing and trusting those who come into it. Building strong and valuable relationships is important to us, and shared interests is a good way to build a rapport that can help put you at ease.

Our Care Professionals are so much more than just care providers – they are consistent and friendly faces, who truly believe in what we do and are dedicated to providing you with first-rate support and care.

4. A brighter tomorrow...

There is always a bit of a bedding-in process as you, your loved one and their Care Professionals get to know each other better, but we’ll work with you and keep in touch at each stage to make sure everything goes smoothly.

You will always know which Sunlight Care Professional is due. This ensures that you, or your loved one, have consistency of care and always know who will be visiting that day.
